Service Agreement

This Service Agreement (“Agreement”) is entered into on [Date], (“Effective Date”) between mypracteaz.com, Vatsalya Software Pvt Ltd, hereinafter referred to as “Service Provider,” and [Client’s Name], hereinafter referred to as “Client.”

  1. Scope of Services: Service Provider agrees to provide Software as a Service (SAAS) to Client for a subscription period of 1 year. The services include but are not limited to OPD EMR, Indoor Module, PRC Module, Group Practice Module, and Enterprise Module.
  2. Subscription Plans: Client agrees to subscribe to one or more plans, including the base plan and additional modules as specified in Exhibit A, attached hereto.
  3. Terms and Conditions for Doctors: Clients acknowledge and agree to abide by the terms and conditions outlined in Exhibit B, specifically tailored for the use of software by doctors.
  4. Terms and Conditions for Patients: Clients acknowledge and agree to ensure that patients using the software comply with the terms and conditions outlined in Exhibit C, specifically tailored for the use of software by patients.
  5. Privacy Policy: Service Provider’s privacy policy, outlined in Exhibit D, governs the collection, use, and protection of personal information. Clients and patients must adhere to this policy.
  6. Refund Policy: Details regarding the refund policy can be found in Exhibit E, outlining the conditions under which refunds may be applicable.
  7. Confidentiality: Both parties agree to keep all proprietary and confidential information confidential and not to disclose it to third parties.
  8. Termination: Either party may terminate this Agreement with written notice if the other party breaches any material term or condition.
  9. Governing Law: This Agreement shall be governed by and construed in accordance with the laws of [Your Jurisdiction].
  10. Miscellaneous: Any amendments or modifications to this Agreement must be in writing and signed by both parties.

IN WITNESS WHEREOF, the parties hereto have executed this Service Agreement as of the Effective Date.
